In short. Change on one side only, bruising without injury, a lump, persistent swelling, or darkness with fatigue and weight change: what each may point to and who to see.

Key points

  • Where any of the above is present, the question stops being which of the four kinds of dark circle this is.
  • The cause has to be established before treatment is discussed, not because treatment is forbidden, but because the timing and the choice of treatment depend entirely on what is producing the change.

Almost every dark circle is a cosmetic concern and the assessment is unhurried. A small number are the visible part of something else. The difference is not made by how dark the area is, but by how the change arrived and what accompanies it.

Change on one side only

Inherited pigment, ageing and the ordinary structural changes of the face are broadly symmetrical, even when they are not perfectly matched. Darkness clearly present on one side and absent on the other has a longer list of possible causes, and it should be assessed medically rather than treated cosmetically. This is the single most important finding on this page.

Bruising without an injury

Blood appearing under the skin without a remembered injury, and particularly without any cause, is not a cosmetic finding. It may relate to medicines, to a bleeding tendency, or to something else that needs investigating. It is a reason to see a doctor promptly rather than to accept camouflage for it.

A lump, or swelling that does not settle

A discrete lump, a swelling that is persistent rather than variable, swelling with pain, redness or warmth, or an eye that appears to be bulging forward all need assessment. None of them is identified by looking in a mirror.

Findings that change the whole plan

Where any of the above is present, the question stops being which of the four kinds of dark circle this is. The cause has to be established before treatment is discussed, not because treatment is forbidden, but because the timing and the choice of treatment depend entirely on what is producing the change.

Darkness with fatigue, weight change or breathlessness

Iron deficiency, thyroid disease and other general conditions change how the area looks, and are worth knowing about for their own sake rather than for the darkness. Darkness arriving with tiredness, breathlessness, heavy periods, weight change or a change in appetite belongs with a physician first. That assessment is not a delay in cosmetic treatment; it decides whether cosmetic treatment is relevant at all.

Pain, vision and the eye itself

Pain on moving the eye, double vision, any change in vision, a pupil different from the other side, or an eye that has become red and painful makes the assessment urgent rather than routine. These are not features of an ordinary dark circle, and emergency care is the right route.

See a doctor before any cosmetic plan

  • Darkness on one side only, or a change that has developed over weeks rather than years.
  • Bruising without injury, a lump that can be felt, or swelling that does not settle.
  • Darkness with fatigue, weight change or heavy periods, or any change in vision.

What the ordinary cosmetic kinds are is set out in the four problems behind the name. How each is told apart in an examination is described in telling which kind you have, and the reasons a fill can make the area worse are explained in when filling goes wrong.

Frequently asked questions

How would I know if my dark circles are one-sided?

Compare the two sides in the same light, and cover one side of your face with a hand while you look at the other. Ageing and inherited patterns are broadly symmetrical, so a clear difference between the sides is a finding to take seriously rather than to treat. It is worth mentioning even when the difference is subtle, because the examiner is looking for it.

Could the darkness be a sign of anaemia?

It can accompany it, and iron deficiency is common enough to be worth excluding when darkness arrives with tiredness, breathlessness, heavy periods or a change in appetite. The darkness itself does not diagnose anything. What matters is the combination, which is why that combination belongs with a physician rather than an aesthetic clinic.

What should I do if I am not sure whether this is medical?

Say so when you make an appointment, whichever appointment you make. A clinician can establish within minutes whether what you have is within the ordinary cosmetic range, and a question asked at the start costs nothing. Waiting to see whether it settles is the one approach that carries a real cost when the cause turns out to be something that should have been looked at earlier.

Risks & Limitations

  • Filling a bulge mistaken for a hollow adds weight and shadow rather than removing them, and correcting that is harder than the original complaint.
  • Agents and peels that act on pigment can irritate the skin, and pigment returns or deepens when sun protection is not kept up.
  • Light treatment is given over a series of sessions, and the result is judged weeks later rather than immediately.
  • No treatment removes the shadow cast by the bone rim around the eye, and the area continues to change for unrelated reasons.

When to Seek Care

  • Darkness on one side only, or a change that has appeared over weeks rather than years.
  • Bruising without an injury, a lump that can be felt, or swelling that does not settle.
  • Eye pain, double vision, or any change in vision — seek urgent care.
  • Darkness with fatigue, weight change or heavy periods, which needs a medical assessment.
